Can I get a passport for my child?

If one parent/guardian cannot be present, a signed and notarized Form DS-3053 Statement of Consent for Issuing a Passport to a Child must be provided. Birth certificates must be submitted for all children showing parental relationship, even if the child has had a passport previously. For more information about obtaining passports for children under age 16, please visit travel.state.gov.
